Worker Dies at Ineos Nitrile Texas Plant

A 21-year-old worker at an Ineos Nitriles plant in Green Lake, Texas, died on June 10 from exposure to cyanide. The man identified as James Wade Gutierrez was employed by an Ineos local contractor, Zachry Industrial, based at San Antonio, Texas.
Circumstances of the exposure were under review, the Ineos subsidiary said, adding that it had contacted the US Occupational Safety and Health Administration (OSHA), which is currently investigating the death.
In another Texas chemical worker death, dating back to November 2014, unions representing workers at DuPont’s La Porte facility are calling on the company to accept OSHA fines totaling $99,000.
DuPont is appealing the fines related to the incident in which four workers died in a leak of methyl mercaptan.
In regard to the incident, the chemical giant was cited for 11 violations, one repeat violation, nine serious violations and one "other than serious" violation.
